How to schedule an evaluation or follow up treatment
What to expect
At your initial evaluation you will have a thorough assessment, development and initiation of your treatment plan, and be provided your initial home exercise program.
The initial evaluation will take approximately 60-90 minutes. If a follow up treatment(s) are necessary the next one will be scheduled.
Please complete and sign the medical history forms and consent to treatment located in the forms section or that are emailed to you at least one day prior to your appointment so that the physical therapist can review them in advance to prepare for your evaluation.
